Obtaining a Sacramental Record

If you are not in possession of the candidate's baptismal certificate presented at the time of baptism and the candidate was baptized at St. Mark:

• Contact the parish front desk at 210.494.1606 and submit a request for a copy of the candidate's sacramental record.
• Once the request is fulfilled, you will have to retrieve the record from the front desk as youth ministry staff are not permitted to handle this type of document.
• You may submit a copy of this record to Maritza at mshreffler@stmarkevangelist.com.
• If you present an original certificate to a youth ministry staff member during office hours, we would be happy to make a copy for our files and immediately return the original to you.

If you are not in possession of the candidate's baptismal certificate presented at the time of baptism and the candidate was NOT baptized at St. Mark:

• You must contact the parish at which the candidate was baptized and ask for more information on how you can obtain a new certificate.
